“We were told this was a significant project that needed to be done right now and without any delay,” says fish.
“This was a large project and would require a significant amount of time. An error or oversight would cost millions in a recall.
“At one of the first project meetings, it turned out that two others from different groups had been tasked with the same project previously and independently — and Team B had already done most of the work and needed only a minor update to complete the project.”
